My 4 year old has a temp of 104 and an ear infection vomiting?

My 4 1/2 year old son has an ear infection. he has been to childrens hosp. and to the drs. his fever keeps coming back hes burning up. what do i do??? the ER is scary for kids and they r mean there. i am exhausted!!!!!
